Intake Form

The purpose of an initial consultation is for the attorney to advise you, the prospective client, what may be done for you. The purpose is not to render a definitive legal opinion as it may be impossible to fully assess a matter within the time frame allotted or with the information or documents that you may be able to provide at the initial consultation.

One of three outcomes may follow the initial consultation:

  • You and Lapekas Law mutual agree to the terms of representation (as set forth in a separate Engagement Letter);
  • Lapekas law declines representation; or
  • You decide not to use our services.

The following questions will help us to understand the reason for your request for an initial consultation. Your responses are protected by attorney/client privilege and are held in strict confidence.

Do you have any criminal history? (Including arrests, charges, and criminal investigations, regardless of outcome)
    YesNo

Are you currently working with an accountant?
    YesNo

In the past 10 years, have you filed all required tax returns?
    YesNo

Does your consultation concern a business that you own?
    YesNo

Are we the first law firm/attorney(s) that you have consulted regarding this matter?
    YesNo

In your initial consultation, Lapekas Law, PA will identify the type of case you have, the key issues, and the options you may have under law. You will discuss whether Lapekas Law, PA will represent you, what the representation could involve, and the amount of the initial retainer that would be required.

Lapekas Law, PA charges $525/hour (charged in 1/10 increments) for their legal services. However, we do offer flat-fee arrangements in some circumstances. We will meet with you for an initial 30-minute consultation for a set fee of $300.00 so that you can decide if you would like to proceed without making a significant financial commitment. You may pay the initial consultation fee in cash, check, or with a credit card. You must sign the agreement and pay the initial consultation fee before the initial consultation. At the conclusion of the initial consultation, we can provide you with an Engagement Letter. However, an attorney-client relationship is not created until both you and Lapekas Law, PA sign an Engagement Letter and the initial retainer has been paid.

If you are paying with cash or check, please present it to us at the beginning of the initial consultation. If you are paying by credit card, please do so when scheduling your appointment.

PLEASE READ CAREFULLY & check the box below.

Following your initial interview, if you agree to hire Lapekas Law, PA and we agree to represent you, you will both sign an Engagement Letter. The Engagement Letter will set forth the terms and conditions of representation.

NOTICE: Lapekas Law, PA does not represent you with regard to the matters set forth herein or discussed during your initial consultation until both you and Lapekas Law, PA sign an Engagement Letter.

I acknowledge that I have completed this Client Intake Form to the best of my ability. I also understand that filling out this form does not mean I have not hired Lapekas Law, PA as my attorney.
